Awards Nomination Form
Each year Oklahoma APSE promotes EMPLOYMENT SUCCESS for workers with disabilities and SUPPORT EXCELLENCE for professionals, businesses and organizations through a state-wide awards program. An awards banquet honoring the winners will be held at the 24th Annual Supported Employment Conference, “WORK: Full Steam Ahead”. The Banquet will be held on October 13, 2011 at the Meridian Convention Center in Oklahoma City.
In order to nominate someone for an award please download or open the Nomination Form, fill it out, save it to your computer then attach the saved file to an email and send to JoDavis@bioscorp.com .
